Health Forms

All students, whether full-time or part-time, are required by the University to complete a report of health history form. This form must be completed and on file in order to finalize registration and attend class.

If you will be a full-time student in the Fall, use the Full-time Student Health Form.
If you will be a part-time student in the Fall, use the Part-time Student Health Form.
